ABSTRACT

This study combines self-learning with a set of online and offline teaching and learning methods such as lecture teaching, case discussion and experimental method. In addition, this study designs a five-step blended teaching mode as well as a multi-tool and multi-dimensional comprehensive evaluation system of the whole process. Both the blended teaching mode and the comprehensive evaluation system have achieved good results after the implementation. The chapter test results show that the students' test scores applying this kind of blended teaching mode are significantly higher than that applying the traditional teaching mode; the ability test results show that under the high complexity blended teaching mode, the students' ability has been improved more effectively, which is more conducive to the realization of the core competence goals in the professional training objectives and teaching objectives.

ABSTRACT

Andrographis Herba is a commonly used plant medicine, and has been recorded in pharmacopeias of different countries. However, there are some differences in the quality standards. Based on this, this paper compare the quality standards of Andrographis Herba between Chinese Pharmacopoeia, Hong Kong Chinese Materia Medica Standards, United States Pharmacopoeia, European Pharmacopoeia and Indian Pharmacopoeia, including origin, botanical characteristics, identification(microscopic identification and chromatographic identification), content determination, specific test(such as impurities, loss on drying, extractives, pesticides, heavy metals, mycotoxins, and other items) and storage requirements, so as to provide a reference for studying international quality standards of Andrographis.

ABSTRACT

Neutrophilic granule protein (NGP) was previously reported as a granular protein of neutrophils in mouse, but the function has not been known clearly. We found the presence of the possible signal peptide in NGP and validated this protein is circulating in the bloodstream. In our findings, NGP is being modified post-translationally in Golgi apparatus and endoplasmic reticulum, which is a universal character of secretory molecules with a signal peptide. The secreted NGP protein could be detected both in vitro and in vivo. NGP has sequence similarity with an antimicrobial protein cathelicidin, and we observed the aspect of inflammation of NGP. Interestingly, NGP interacts with the complex of LPS and LPS binding protein (LBP). This interaction blocks the binding of the complex of LPS and LBP to TLR4 and the downstream inflammatory signals. Furthermore, the inhibitory function of NGP against the inflammatory effect of LPS could be observed in both in vitro and in vivo. With these findings, we report NGP is a novel secretory protein to mask LPS and inhibit its function.

ABSTRACT

Objective To analyze the interaction between family history and waist circumference on incidence of hypertension .Methods We conducted a cross-sectional survey of rural residents in Hanzhong .Questionnaires about hypertension were collected and physical examination was conducted .A total of 3030 rural residents' data were collected . Logistic regression model was used to analyze the influences of family history and waist circumference on hypertension . Additive model and multiplicative model were used to analyze the interaction between family history and waist circumference on hypertension .Results For hypertension , Logistic regression analysis showed that family history (OR :2 .225 ,95% CI:1 .843-2 .687) and abdominal obesity (OR :1 .976 ,95%CI :1 .649-2 .368) were positively associated after adjustment for age ,sex ,salt intake ,smoking ,and alcohol drinking .Family history and waist circumference had no interaction on incidence of hypertension on a multiplicative scale but an interaction on an additive scale .The relative excess risk due to interaction (RERI) was 1 .848 (95% CI :0 .693-3 .002) ,the attributable proportion due to interaction (AP) was 0 .404 (95% CI :0 .227 -0 .582) and the synergy index (S) was 2 .073 (95% CI :1 .340 -3 .206 ) .Conclusion Family history and abdominal obesity are risk factors for hypertension . The incidence of hypertension of people with both family history and abdominal obesity is obviously higher than the sum of the two risk factors separately .Therefore ,to prevent hypertension it is more important for people to control their weight when they have a positive family history .

ABSTRACT

Objective To investigate the incidence and influence factors of chronic postoperative inguinal pain (CPIP) after tension-free repair for inguinal hernia.Methods The retrospective case-control study was conducted.The clinicopathological data of 1 188 patients who underwent tension-free repair for inguinal hernia in the People's Hospital of Xinjiang Uygur Autonomous Region between January 2011 and August 2017 were collected.Observation indicators:(1) surgical and postoperative situations;(2) follow-up;(3) influence factors analysis of CPIP.Follow-up using outpatient examination and telephone interview was performed to detect inguinal pain and complications at 1,3,6 months and 1 year postoperatively up to August 2018.Measurement data with normal distribution were represented as x ±s.Measurement data with skewed distrubution were described as M (range).The univariate and multivariate analysis were done using the chi-square test and Logistic regression model.Results (1) Surgical and postoperative situations:1 188 patients underwent successful tension-free repair for inguinal hernia.Eighty-five patients (7.155%,85/1 188) had CPIP,including 76 (8.370%,76/908) undergoing open surgery and 9 (3.214%,9/280) undergoing laparoscopic surgery.(2) Follow-up:1 188 patients were followed up for 12-36 months,with a median time of 19 months.Incidence rates of CPIP with visual analogue score > 3were 11.785%(140/1 188),7.155%(85/1 188),5.808%(69/1 188),3.199%(38/1 188) at 1,3,6 months and 1 year postoperatively.Patients were given individualized and reasonable treatment according to their own conditions and CPIP was relieved after conservative treatment including drug treatment,physiotherapy such as acupuncture,nerve block and psychotherapy.Of 5 patients with CPIP after tension-free repair for inguinal hernia undergoing surgeries,1 was relieved nerve ligation by surgery,3 with mesh related pain were removed meshes,1 was taken the fixed stiches out.They were relieved CPIP after above treatments.During the follow-up,161 patients with incisional seroma,75 with incisional infection and 5 with disruption of wound were cured by symptomatic treatments including reinforced incision management,dressing change and physiotherapy.Seven patients with mesh infection were removed meshes.Of 68 patients with hernia recurrence,53 had reoperation,18 complicated with diseases induced severe increased intra-abdominal pressure were suggested to undergo surgeries after treatment of complications.(3) Influence factors analysis of CPIP:① results of univariate analysis showed that sex,age,bodymass index,surgical method,degree of preoperative pain,intraoperative nerve stretching or injury and incisional infection were related factors affecting CPIP after tension-free repair for inguinal hernia (x2 =21.002,6.715,6.012,8.563,11.887,49.447,10.025,P<0.05).② Results of multivariate analysis showed that sex,body mass index,surgical method,degree of preoperative pain,intraoperative nerve stretching or injury and incisional infection were independent related factors affecting CPIP after tension-free repair for inguinal hernia (odds ratio =1.267,2.986,1.661,3.208,2.034,1.871,95% confidence interval:1.042-1.392,1.372-4.901,0.998-2.758,1.933-6.013,1.556-3.118,1.095-3.534,P<0.05).Conclusions Sex,body mass index,surgical method,degree of preoperative pain,intraoperative nerve stretching or injury and incisional infection are independent related factors affecting CPIP after tension-free repair for inguinal hernia.Preoperative pain management and psychological counseling,intraoperative refine performance,inguinal nerve protection and postoperative incisional management should be reinforced to prevent and reduce incidence of CPIP.

ABSTRACT

Objective To investigate the imaging features of acute interstitial pneumonia and to provide the reference for early diagnosis and treatment.Methods The imaging and clinical data of 50 patients with clinically diagnosed acute interstitial pneumonia in our hospital were analyzed retrospectively.Results X-ray film showed the fan-shaped and lightly patchy shadows around the lung hilum distributing along the lung markings in multiple lung fields,accompanied by thickening of interlobular septa with varying degrees and thickened bronchial vascular shadows running through the above-mentioned light patchy shadows.CT revealed the fan-shaped ground glass opacity in the pulmonary area with rich blood supply,interspersed with thickened clustered pulmonary arteries and veins.Conclusion The imaging features of acute interstitial pneumonia have certain characteristics.The suggestive diagnosis based on imaging plays an important guiding role in early clinical diagnosis and treatment.

ABSTRACT

<p><b>BACKGROUND</b>Adenoid hypertrophy (AH) is associated with pediatric chronic rhinosinusitis (pCRS), but its role in the inflammatory process of pCRS is unclear. It is thought that innate immunity gene expression is disrupted in the epithelium of patients with chronic rhinosinusitis (CRS), including antimicrobial peptides and pattern recognition receptors (PRRs). The aim of this preliminary study was to detect the expression of innate immunity genes in epithelial cells of hypertrophic adenoids with and without pCRS to better understand their role in pCRS.</p><p><b>METHODS</b>Nine pCRS patients and nine simple AH patients undergoing adenoidectomy were recruited for the study. Adenoidal epithelium was isolated, and real-time quantitative polymerase chain reaction (RT-qPCR) was employed to measure relative expression levels of the following messenger RNAs in hypertrophic adenoid epithelial cells of pediatric patients with and without CRS: Human β-defensin (HBD) 2 and 3, surfactant protein (SP)-A and D, toll-like receptors 1-10, nucleotide-binding oligomerization domain (NOD)-like receptors NOD 1, NOD 2, and NACHT, LRR and PYD domains-containing protein 3, retinoic acid-induced gene 1, melanoma differentiation-associated gene 5, and nuclear factor-κB (NF-κB). RT-qPCR data from two groups were analyzed by independent sample t-tests and Mann-Whitney U-tests.</p><p><b>RESULTS</b>The relative expression of SP-D in adenoidal epithelium of pCRS group was significantly lower than that in AH group (pCRS 0.73 ± 0.10 vs. AH 1.21 ± 0.15; P = 0.0173, t = 2.654). The relative expression levels of all tested PRRs and NF-κB, as well as HBD-2, HBD-3, and SP-A, showed no statistically significant differences in isolated adenoidal epithelium between pCRS group and AH group.</p><p><b>CONCLUSIONS</b>Down-regulated SP-D levels in adenoidal epithelium may contribute to the development of pCRS. PRRs, however, are unlikely to play a significant role in the inflammatory process of pCRS.</p>

ABSTRACT

BACKGROUND:Traditionaly repairing single vertebral thoracolumbar fracture often uses of cross-vertebral 4 screw fixation. Because of “paralelogram effect” and the “suspension effect”, it is prone to have internal fixation failure, reset height and correction angle loss, and other complications. However, these complications can be avoided or reduced by fractured vertebra pedicle screw fixation. OBJECTIVE:To investigate the feasibility evaluation of pedicle screw fixation for repairing thoracolumbar fracturevia fractured vertebrae. METHODS:A total of 23 patients with single segment thoracolumbar fractures treated at the First Hospital of Lanzhou University between January 2013 and June 2014 were retrospectively analyzed. The fractured segments were between T11 to L2, the patient’s average age was 47.8 years old. Al the patients received the posterior pedicle screws and adjacent and up and down six pedicle screw internal fixation within 10 days after injury. The repair effect was observed, including short-term postoperative complications, vertebral height ratio, Cobb angle and the recovery of spinal neurological function. RESULTS AND CONCLUSION:Twenty-three cases were folowed up for 4 to 15 months. Al patients had no severe complications in the short term (4 to 15 months) after treatment. There was no screw rod breakage or internal fixation loosening at the last folow-up. The vertebral height ratios and Cobb angle which were measured in postoperative and final folow-up were significantly improved compared with the preoperative data (P 0.05). The spinal neurological functions of most patients were partialy recovered at the last folow-up. These results suggest that pedicle screw embedding fixation repairing thoracolumbar fractureviafractured vertebrae can effectively restore vertebral height and correct kyphosis, and maintain correction effect. The advantages are obvious with a low occurrence rate of pedicle screw fracture.

ABSTRACT

<p><b>OBJECTIVE</b>To express HPV31 and 52 L2 fusion protein and detect its immunogenicity.</p><p><b>METHODS</b>According to the amino acid sequences of HPV31 and 52 L2 11-200AA published in the GenBank database, weartificially synthesized the HPV31 and 52 L2 fusion gene which was optimized according to Escherichia coli codon usage and encodes 11-200 amino acid of HPV31 and HPV52 L2, then cloned it into pET-9a vector. The HPV31 and 52 L2 fusion protein was expressed in Prokaryotic expression system and the mice were immunized with the fusion protein after purification. The immunogenicity was characterized in vaccinated mice.</p><p><b>RESULTS</b>HPV31 and 52 L2 fusion protein was highly expressed in E. coli, the amount of fusion protein is nearly 20% of the total bacterial protein. The purified fusion protein with aluminum adjuvant could induce specific high titer of IgG antibodies detected by ELISA, and also induce the neutralizing antibodies against pseudovirus of HPV31 and HPV52 and cross-neutralizing antibodies against pseudovirus of HPV45, 58, 16, 18.</p><p><b>CONCLUSION</b>HPV31 and 52 L2 fusion protein could induce neutralizing and cross-neutralizing antibodies against HPV pseudovirus. It provides laboratory basis for development of HPV L2 protein vaccine.</p>

ABSTRACT

<p><b>OBJECTIVE</b>Comparative and statistical analysis the HPV infection rate between fresh tissue and Paraffin-embedded Specimens of esophageal squamous cell carcinoma,and comparative the testing results with others regions.</p><p><b>METHODS</b>Extracted the total DNA from the novel fresh tissue and Paraffin-embedded Specimens; Detected the DNA by PCR with universal primer and Detected the HPV type with human papilloma virus nucleic acid amplification-based typing detection reagent kit (Hybribio); Compared the statistical result from the different specimens; analyzed the result between different region.</p><p><b>RESULTS</b>HPV infection rate of fresh tissue is 82.6% with HPV16 (34.8%) and HPV18 (34.8%), and paraffin-embedded specimens is 78.2% with HPV16 (30.4%) and HPV18 (17.4%).</p><p><b>CONCLUSION</b>The results provides the first evidence that there wasn't noticeable difference between HPV infection rate of the two specimens. So broader specimen source could be used for HPV testing.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To compare the efficacy and safety of domestic levosimendan versus dobutamine for patients with acute decompensated heart failure (ADHF).</p><p><b>METHODS</b>ADHF patients from 8 medical centers were recruited in this multicenter, blind, positive-controlled, randomized study and received 24 h intravenous levosimendan (n = 114) or dobutamine (n = 114) therapy. SWAN-GANZ catheter was performed in patients with pulmonary capillary wedge pressure (PCWP) ≥ 15 mm Hg (1 mm Hg = 0.133 kPa) and cardiac index (CI) ≤ 2.5 L·min(-1)×m(-2) (n = 39 each).</p><p><b>RESULTS</b>Compared with baseline level, LVEF increased [(31.56 ± 9.69)% vs. (28.44 ± 7.08)%, P < 0.01] at 24 h in both groups. LVEF increase at 24 h was similar between two groups [(3.11 ± 6.90)% vs. (3.00 ± 6.63)%, P > 0.05]. The PCWP decrease at 24 h was significantly greater in levosimendan group than in dobutamine group [(-8.90 ± 7.14) mm Hg vs. (-5.64 ± 6.83) mm Hg, P = 0.04]. Decrease in NT-proBNP at 3 days was also more significant in levosimendan group than in dobutamine group [the percentage change compared to baseline: (-22.36 ± 38.98)% vs. (-8.56 ± 42.42)%, P < 0.01]. Dyspnea improvement at 24 h was more significant in levosimendan group than in dobutamine group. The incidences of adverse reactions and events were similar between two groups.</p><p><b>CONCLUSION</b>LVEF improvement is similar between dobutamine and domestic levosimendan while greater decreases in PCWP and NT-proBNP are achieved with domestic levosimendan in patients with ADHF.</p>

ABSTRACT

Objective To explore the correlation of high-sensitivity C-reactive protein (hs-CRP) with risk factors and target organ damage in hypertensive patients.Methods The levels of serum hs-CRP of 216 hypertensive cases (hypertension group) and 36 healthy subjects (control group) were tested and compared among different associated diseases, the number of involved target organ and the difference of involved target organ.The relativity between variables such as total cholesterol, high density lipoprotein cholesterol (HDL-C), left ventricular mass index (LVMI) and so on and hs-CRP was analyzed by linear correlation analysis and multiple linear regression analysis.Results The levels of serum hs-CRP in hypertension group were higher than those in control group[( 1.99 ± 0.34) mg/L vs.( 1.10 ± 0.26 ) mg/L](P < 0.01 ).The levels of serum hs-CRP in hypertension combined with coronarv disease and hvoertension combined with diabetes mellitus[(2.39 ± 0.24), (2.10 ± 0.18 ) mg/L, respectively]were higher than those in simple hypertension[( 1.85 ± 0.30 ) mg/L], and the levels of serum hs-CRP in hypertension combined with coronary disease were higher than those in hypertension combined with diabetes mellitus, and there were significant difference (P < 0.05 ).The levels of serum hs-CRP were positively correlated with the number of involved target organ (r =0.747,P <0.01 ).There were significant differences among different associated diseases.The levels of serum hs-CRP in hypertension combined with left ventricle thickening were higher than those in hypertension combined with carotid atherosclerosis, renal damage and diabetic retinopathy,and there were significant differences (P < 0.05 ).There was no significant difference in the level of serum hs-CRP between hypertension combined with carotid atherosclerosis and hypertension combined with renal damage (P > 0.05 ).Stepwise regression analysis showed that the dominating factors of the level of serum hs-CRP were LVMI, age and HDL-C, and the level of hs-CRP showed negative correlation with HDL-C.Conclusions The levels of serum hs-CRP in hypertensive patients are higher than those in healthy subjects.The more number of involved target organ, the higher levels of serum hs-CRP.Patients with different involved target organ have different inflammatory degree.The levels of serum hs-CRP in hypertension combined with coronary disease are higher than those in hypertension combined with diabetes mellitus.Stepwise regression analysis shows that the dominating factors for hs-CRP levels are LVMI, HDL-C and age.

ABSTRACT

Objective To investigate the relationship between low-grade albuminuria and unfavourable cardiac geometric adaptations and left ventricular diastolic dysfunction in non-treated essential hypertensive(EH ) patients. Methods Three hundred and one patients of EH were divided into three groups acording to morning urine albumin concentration ( MU AC ): normoalbuminuria (NAU) group ( 107 cases,MUAC < 10 mg/L) ;low-gradealbuminuria (LGAU) group ( 127 cases, MUAC 10-30 mg/L); microalbuminuria (MAU) group (67 cases, MUAC > 30 mg/L). The blood pressure,body height and weight was measured for all the patients. The serum creatinine (SCr),blood urea nitrogen (BUN), uric acid (UA),fasting plasma glucose (FBG), plasma glucose 2 hours after oral glucose load (2hPG), total cholesterol (TC), triglyceride (TG) and low density lipoprotein cholesterol (LDL-C) were detected by automatic biochemical analyzer.Echocardiogram exarmination was performed to detect left ventricular end-diastolic diameter (LVEDD), left ventricular end-diastolic posterior wall thickness (LVPWT) and interventricular septum thickness at end-diastole (IVST). Color Doppler ultrasound was used to measure indicators of left ventricular diastolic function, including peak E velocity of mitral diastolic flow (E) and peak A velocity of mitral diastolic flow ( A ), calculated E/A, measured the deceleration time of peak E ( DT ), and isovolumic relaxation time (IV RT ).Left ventricular mass index (LVMI) and relative wall thickness(RWT) were calculated by formula, and LVMI,RWT,left ventricular configuration diastolic dysfunction was compared. Results The level of MUAC had positive relationship with LVMI (r = 0.43 ,P < 0.01 ). Compared with that in NAU group, IVST, LVPWT,RWT, LVMI increased (P < 0.01 ), the ratio of left ventricular normal configuration decreased (P < 0.01 ), the incidence of concentric reconstitution, concentric hypertrophy increased (P < 0.05 ), A increased (P < 0.05 ),and E/A decreased (P < 0.01 ) in LGAU group. The total incidence of left ventricular diastolic dysfunction was 28.6%(86/301 ),12.1%(13/107),33.9%(43/127) and 44.8%(30/67)in NAU group,LGAU group and MAU group. The incidencerates of left ventricular diastolic dysfunction in LGAU group and MAU group were higher than that in NAU group (P < 0.01 ). Conclusion LGAU is positively related to incidence of unfavourable cardiac geometric adaptations and left ventricular diastolic dysfunction in EH patients.

Objective To explore what kind of role serum interleukin 6 (IL-6) and interleukin 18 ( IL-18) play in the relationship between type D personality and acute coronary syndrom (ACS)prognosis.Methods Serum levels of IL-6,IL-18 in all 214 ACS patients were measured with ELISA.According to the scores of type D personality scale(DS14) ,the subjects were divided into the group with type D personality (50 cases)and the one without type D personality ( 164 cases).The serum IL-6 and IL-18 level between the two groups were detected and analyzed.Results There were 23.36% ACS patients with type D personality.The serum IL-6, IL-18levels of the group with type D in ACS acute stage and ACS recovery stage were (2.340 ± 0.081 )OD, (2.016 ±0.023 ) OD and ( 1.460 ± 0.070 ) OD, ( 1.313 ± 0.012 ) OD respectively, significantly higher than those of the group without type D (2.178 ± 0.180)OD, ( 1.849 ± 0.159)OD and ( 1.387 ± 0.091 )OD, ( 1.196 ± 0.132 ) OD respectively(P<0.01).Conclusion At home and abroad ,there are similar amount of ACS patients with type D personality.The high serum IL-6 and IL-18 levels may play an important role in pathogenesis of relationship between type D personality and ACS poor prognosis.

ABSTRACT

Objective To investigate the incidence of target organ damage in prehypertensive. Methods Two hundred and seventy-eight prehypertensive cases (prehypertensive group) and 136 normotensive cases(normotensive group) were involved in this study. The blood pressure,body height,weight and waist circumference were measured. Serum creatinine (SCr), urea nitrogen (BUN),uric acid(UA), fasting blood glucose (FBG), total cholesterol (TC), triglyceride(TG),low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C) and morning urine albumin concentration were detected by automatic biochemical analyzer. Left ventricular end-diastolic dimension (LVEDd), left ventricular posterior wall thickness (LVPWT), interventricular septal thickness (IVST), intima-media thickness (IMT) and plaque index (PI) were examined by ultrasonography. The incidence rates of plaque, left ventricular mass index (LVMI), relative wall thickness (RWT), glomerular filtration rate (GFR) and body mass index (BMI) were calculated. Results The proportion of ≥ 1, ≥2 and ≥3 kinds of risk factors was 71.9% (200/278),40.6%(113/278) and 13.7%(38/278) in prehypertensive group and 30.1%(41/136), 13.2% (18/136) and 5.9%(8/136) in normotensive group, and there was significant difference between the two groups (P<0.05). Compared with normotensive group, the age,systolic pressure, diastolic pressure, FBG, waist, BMI, morning urine albumin, TG,TC,UA,LDL-C,IVST,LVPWT,RWT,LVMI,IMT,PI were higher, meanwhile the incidence rate of concentric reconstruction of left ventricular, concentric hypertrophy, eccentric hypertrophy and carotid plaque was higher, and the incidence rate of left ventricular normal configuration and GFR was lower (P<0.01 or <0.05). Conclusion People with prehypertensive may accompany with many kinds of cardiovascular risk factors, and the incidence rate of target organ damage is significantly increased.

ABSTRACT

Objective To investigate the possible mechanisms of ox-LDL on vascular smooth muscle cells (VSMC) with and without exogenous zinc finger protein A20 gene. Methods VSMC were trans-fected with plamid containing A20 gene and incubated with ox-LDL. TLR4 and LOX-1 mRNA expression were measured by RT-PCR. Results After VSMC incubated with ox-LDL, the expression of LOX-1 and TLR-4 mRNA reached a higher level. However, when VSMC were transfected with plasmid containing A20 gene, the NF-kB nuclear translocation and the ability of migration reduced to normal level. Conclusions ox-LDL can significantly activate NF-kB signaling system and enhance migration in VSMC. The activation may induce the inflammatory response in arterial wall, the migration of VSMC to the intima, and triggering the process of atherosclerosis. Transfecting plasmid containing A20 gene significantly inhibited the activation of NF-kB and the ability of migration of VSMC. A20 effects might be through inhibiting NF-kB signaling. Meanwhile, A20 blocked the development of atherosclerosis.

ABSTRACT

Objective To discuss the relationship between pulse pressure and blood uric acid and the effect of simvastatin on pulse pressure and blood uric acid in aged primary hypertension (PH) patients with normal cholesterol.Methods Eighty-five patients with PH were divided into two groups randomly,routine group (using drugs only to lower blood pressure) and simvastatin group (simvastatin was added).Six months later,blood uric acid and pulse pressure was measured again.Results With the development of hypertension,there was statistical difference on pulse pressure and blood uric acid between hypertension grade 3 and grade 1 or grade 2 (P < 0.05).With the increase of pulse pressure,the trend of blood uric acid increase was distinct,and blood uric acid level was positively related with pulse pressure by correlation analyses(r = 0.87,P< 0.05).The pulse pressure and blood uric acid was decreased after treatment compared with that before treatment in simvastatin group (P < 0.05),but there was no significant difference before and dter treatment in routine group (P >0.05).There was significant difference after treatment between two groups (P< 0.05).Conclusions With the increasing of pulse pressure,there is a significant raise in blood uric acid.Blood uric acid is positively related with pulse pressure by correlation analyses.Simvastatin is a drug used to lower cholesterol,and it could also be used to reduce pulse pressure and blood uric acid in aged PH patients with normal cholesterol.

ABSTRACT

Objective To investigate the effect of intensive lipid-lowering therapy on the imbalance between inflammatory and anti-inflammatory responses in patients with acute coronary syndrome (ACS).Methods Observed serum levels of hs-CRP and IL-IO in 82 patients with ACS, 17 patients with stable angina, and 22 controls. Forty-one patients with ACS were randomized to take either atorvastatin 10 mg/d (standard lipid-lowering therapy) or atorvastatin 40 mg/d (intensive lipid-lowering therapy) for one month in addition to their routine anti-anginal treatment. Serum levels of hs-CRP, blood lipids, IL-10 were investigat-ed.IL-10 was measured by ELISA. Results The level of hs-CRP in patients with ACS [(11.10 ± 14.30)mg/L] was higher than that in patients with stable angina [(2.47 ± 2.72) mg/L]and controls [(2.34 ± 4.22)mg/L] (P all < 0.05 ). The level of IL- 10 was lower in ACS patients [( 3.94 ± 1.91 ) ng/L] compared with those who had stable angina [(6.31 ± 4.26) ng/L] and controls [(7.76 ± 2.82) ng/L], Pan <0.05. The level of hs-CRP in patients with ACS was decreased and IL-10 was increased after one month treatment with atorvastatin (P < 0.05).The effect of atorvastatin 40 mg/d was more effective than that of atorvastatin 10 mg/d.Conclusions Patients with ACS have higher level of hs-CRP and lower level of IL-10 than those with sta-ble angina. This finding suggests that imbalance of inflammatory and anti-inflammatory responses is related with aggravation of atherosclerotic disease. Intensive lipid-lowering therapy is more effective than standard lipid-lowering therapy on ameliorating the imbalance.

AIM: Nucleotide binding oligomerization domain receptor 1 (NOD1) is a recently identified intracellular pathogen pattern recognition receptor of innate immunity. This study was designed to explore the role of NOD1-mediated innate immune signal pathway in the activation of vascular smooth muscle cells (VSMCs), and investigate the effect of peptidoglycan (PGN) on the activation and expression of NOD1 in human VSMCs. METHODS: The experiment was performed in the central laboratory of the Second Affiliated Hospital of Dalian Medical University from June 2006 to March 2007.①Subject: Human coronary artery VSMCs were purchased from Cambrex company.② Methods: Human coronary artery VSMCs were cultured in vitro, and stimulated with NOD1 agonist PGN (10 mg/L) for 0, 3, 6 and 24 hours. ③ Evaluation: The mRNA expression of NOD1 in VSMCs was measured by real time quantitative reverse transcription-polymerase chain reaction.
